Then please post you exact encoding settings for the trouble some stream.
If really ALL three servers are exactly the same for all other settings...but only the one is causing trouble it must be somewhere in the encoder settings...

Another user just experienced the same issue. It seems to be a new Microsoft bug.
But a workaround ia already available here.
