That's exactly the same options as I am using (I just have also enabled the 'EventEntries' with the 'What MetaData oto Save' option - but that should be irrelevant).
So I am lost why it is not working on your side, as you can see it is woking here just fine.
So some stupid questions:
- you are sure, that you have updated the version
- have you tried a reboot

The 'about' windows does show v4.2.1.0. I've just now rebooted and retried but that didn't work either.
The problem appears to be completely cartwall related: I've opened call 01 from the cartwall in PFL, changed the next cue, clicked 'Save to metadata tag', closed PFL and reopened call 01 from the cartwall: cue is wrong again.
Could this be a pointer?

Can you check, if the file is really accessible, i.e. might it be 'write-protected'?
Else I wouldn't have any other explanation.
I also checked with a few other customers and its all working with them!

I've copied the files off my NAS to my local PC and moved them from there into a new folder structure. After that I've renamed the files, to exclude both permission and path/naming problems. The local Windows user account under which PFOA runs has full control permissions on the files and they are not write protected or read-only.
I've started PFOA (not via right click, run as administrator, but as a regular user, like I normally would). I've added the new, local folder to the directory browser in PFOA and opened the first track from there in PFL: The next cue point is correct.
I've now created a new folder based cartwall library for this new folder. I open the first track from cartwall I in PFL and it has a completely different Next cue point. They should be equal right? Or does PFOA perform some sort of automatic cue point detection from within the cartwall? Or perhaps some sort of caching?
Could the very short duration of these jingles have something to do with it?

Have you copied the entire folder or the individual audio files only - I hope the later.
All that sounds very weir and honestly I have never heard of something like this!
I even tested the same here on 3 different Test Systems - no problem and all is correct as expected.
So there might be something wrong with your system, permission or file!
ProppFrexx however does NOT cache anything!
TAG and meta data are ONLY read from the audio file's TAG data, the .pfmd file or a related .pfp playlist file.
ACPD is not happening if cue-points are already present.
Please send two screenshots:

2. make the same on the directory browser...
Make sure the path points to exactly the same physical file.

Yes, that proves, that there must be either a wrong _synced_.pfp which was also copied or .pfmd files.
Could they maybe hidden or something like that?
Else this is physically impossible.
