The log file (if you use the standard way) is written with the global even 'Playlist.OnTrackPlay' even, see the general settigns.
So I assume, the track was may be played 4 times that day!
I noticed (during looking at your system via TeamViewer), that you have quite some tight filter rules, which sometimes only results in a couple of possible result entries - together with the gloabl AND script history - this might pretty much limit the choices.
So I don't see, why this is essentially wrong?!
Might be, that your filters are just too tight? E.g. 'Random' out of 100 might result in repeats...?

The script at that time has no "Random" rule.
It has "LeastRecentlyPlayed". That's why I am worried.

Look at the screenshot.
A song plays at 16/04/2016 19:28.
Later I try to find this song in Find Track Window. It shows that this song played last 05/04/2016 17:15 Unless I am wrong, it should display 16/04/2016 19:28

Please try the new v4.0.1.25.
It fixes a bug with multiple Entry Statistics - which is the case when using the entry stats also on the MLS, as in your case.
In the previous versions I did not consider the most up-to-date stats entry - as normally they should be in sync, but the previous meta data bug, they are not.
